Our primary care physicians provide general healthcare for overall well-being, treatment for acute conditions like colds and allergic reactions, and long-term management of chronic diseases like diabetes and heart health. Our doctors also provide referrals to specialist consultants as and when needed.
If you are a Medicare beneficiary, you are entitled to one free Medicare Annual Wellness Visit per year, and we strongly recommend you make good use of this benefit. An additional yearly physical exam keeps your care provider up to date with your changing healthcare needs, and lets them create effective preventative care plans for you.
Primary care doctors are your first point of contact in long-term care for overall health. They help you manage chronic illnesses, treat acute illnesses, prescribe medications, and perform ear cleanings and minor wound care. Specialists focus on specific conditions, for example a cardiologist for advanced heart problems.
Yes! We provide personalized weight management plans and mental health support, including counseling and screenings, to help you feel your best.
All our clinics are equipped to accommodate patients with urgent, sudden on-set illnesses such as colds, infections, and minor injuries. Please call or book online to check the availability of a suitable care provider at your nearest location.
Please bring:
- A government-issued photo ID such as a driver’s license or passport.
- Your Medicare/Medicaid/private health insurance card.
- A list of all medications you currently take, and any medications you have taken in the past year. These include prescription drugs, over-the-counter medications, vitamins, and any other supplements.
- Notes on your medical history so that you don’t forget to mention anything important.
- Record of vaccinations and allergy information.
- Emergency contact information (name, phone number, email address, and postal address).

Please wear comfortable, loose-fitting clothing with sleeves that can be rolled all the way up to your shoulders, and shoes that can be easily slipped off.
