For diabetes management at IntraCare Health Centers, your primary care physician’s objective is to ensure your health remains stable and the medication and/or dosage levels of insulin prescribed to you are effective. Your doctor will keep a close eye on your weight and nutrition and will adjust your medication and/or insulin dosages as needed.

Weight Management
Your primary care physician will regularly evaluate your diet and draw up nutrition plans to help you achieve a healthy weight. They will also advise on the best exercises for you.
Blood Glucose Monitoring
Your care provider will have you note your blood glucose levels at various times of the day. They will use this information to calculate how much insulin you need and at what time.
Comorbidity Checks
Diabetes can lead to other serious illnesses, or comorbidities. Your care provider will diligently monitor your health to catch and treat any new illness at its earliest stage.

Senior patients are usually diagnosed with type 2 diabetes. This kind of diabetes can be brought on by genetics, but the more common reasons are being obese and physically inactive.
Your care provider will have you check your blood sugar regularly, advise on diet, nutritition, and exercise, and keep an eye on other illnesses that may develop as consequence of diabetes, such as cardiovascular disease, kidney failure, and glaucoma.
You may be required to test your blood sugar at regular times during the day and note the readings on a chart. If your doctor has issued a remote patient monitoring (RPM) glucometer, your readings will be sent automatically to their health records system.
People with diabetes are at higher risk of gangrene, especially of the feet, but with proper care and timely attention it is possible to reduce the risk of complications.
Not necessarily. Following your doctor’s guidelines on eating well and exercise, keeping your blood sugar under control, and quitting smoking may help you protect your eyesight for years to come.
